Employee pinned under forklift

At approximately 10:30 a.m. on May 22, 2003, Employee #1 was employed as a warehouse worker by 7UP RC through a temporary agency. His job duties included separation of returned products and general clean up. On the day of the incident, Employee #1 was driving an electric powered forklift to dump a bin filled with trash. After he raised the attached bin and dumped the trash into the big bin which is located outside of the main production building, Employee #1 backed up the forklift. The left rear wheel of the forklift hit a pothole, causing the forklift to lose balance and tipped over to the left side, crushing his right leg between the cage and the asphalt driveway. Employee #1 sustained fracture of right femur bone and severe damage to right leg's arteries and veins. He was hospitalized for his injuries.
