Investigation Summary
At approximately 10:00 a.m. on April 15, 2003, Employee #1 and two other employees of a furniture distribution company, finished unloading material in a back storage area of the warehouse and needed to go back to the front. Employee #1 and the second employee were riding on a plastic pallet on the forks of the forklift. The forklift driver, who is also a warehouse manager, proceeded to drive to the front of the warehouse area at approximately 15 to 25 mph. The forklift and platform struck a pole causing employees to be thrown off the platform and onto the cement floor. Employee #1 was transported to Los Angeles County USC Medical Center where he was treated and hospitalized with a fractured left leg. The second employee was treated for injuries to his back, waist and right foot but was not hospitalized. The forklift was a Nissan, Model Number 20, Serial Number PJO2 PJO2A20 PJ02-001042.
