Investigation Summary
At approximately 3:00 p.m. on February 9, 2003, Employee #1, a machine operator, was asked to assist another employee in the cleaning of the masa pump machine. They were told to dissemble the pump by removing the covers of the pump and removing masa from the augers. When he reached inside to remove the masa, his right middle and ring fingers came in contact with the moving parts and were amputated. He was taken to Kaiser Permanente, where he was treated for his injury.
