Investigation Summary
At approximately 1:30 p.m. on May 23, 2002, Employee #1, an employee of L & L Wholesale, was working as a general laborer filling orders in the warehouse. He had been employed with the company for 6 months. On the day of the accident, he was filling an order and needed to retrieve general merchandise off the shelves. He needed to get a product from the second shelf on rack L2. He climbed on the first rack which was 7 feet high to get to the second shelf. He then placed his left hand under the horizontal beam of the second shelf and his right hand on top of the pallet on the second shelf and he stepped on top of the horizontal beam of the second shelf. When he tried to pull himself up onto the second shelf, the horizontal beam that he was hanging on to somehow detached from its connection points, causing him to fall and land on his back on the concrete floor. The employee sustained lower back pain, a sprained right knee and pain in the left forearm. He was transported to the local clinic by a coworker and was released within a few hours.
