Investigation Summary
At 12:16 p.m. on January 21, 2002, an accident occurred in Arcadia, California. The employer is a commercial building electrical contractor who also installed sound and data communication systems. The accident occurred in front of the Starbucks counter at the Pavillions Store Number 3208, where Employee #1 was running overhead cable. Employee #1 was in the process of pulling overhead data cable which ran along the ceiling beam. Employee #1 had been with the company for seven years and currently holds a position of foreman. Employee #1 is an experienced sound installer through hands on training from the previous employer, basic courses in sound, electrical, and continual product upgrade training provided by the current employer. On the day of the accident, Employee #1 was drilling a hole in the drywall to feed the T-1 cable through. Employee #1 used the portable personnel upright lift to pull the cable along the ceiling joists. The outriggers were not installed on the portable personnel lift as instructed by the manufacture's written instruction prior to elevating the lift. As he lowered the upright lift while he was inside the basket, he noticed that the basket was going to hit the suspended chain supporting the fluorescent lights. Employee #1 grabbed the big overhead pipe with his left hand to position the basket and avoid the basket hitting the chains as he was lowering the upright personal lift. This caused the upright lift to tip over. Employee #1 was thrown out of the basket as the upright lift hit the floor. The height measured from the floor to the cable was 22.47 feet. As a result of the accident, Employee #1 sustained a broken right ankle bone and a fractured hip bone and was hospitalized.
