Investigation Summary
At approximately 6:23 p.m. on February 14, 2002, Employee #1 was operating the cup-filler machine Model Number IP 2X4, Serial Number AP-1657 at the cup line station. At the time of the accident Employee #1 attempted to remove a 4-ounce salsa cup that got stuck inside the cup-filler machine with her right hand. The metal bars that push the salsa cups out of the machine caught Employee #1's right hand fracturing her right elbow. Employee #1 suffered fractures, dislocations and crush injuries to her right forearm and hand. Employee #1 was hospitalized.
