Investigation Summary
At 11:30 a.m. on December 24, 2001, Employee #1 was driving an electric-powered stock chaser underneath an overhead storage rack. When he reversed the stock chaser, its ladder struck a horizontal rack beam, the beam broke, and the rack and other products fell on him. Employee #1 was hospitalized with a fracture, and dislocation of his left hip. The contributing cause of the accident was the weight of the materials stored on the rack which apparently caused the beam to deflect and reduce the available clearance.
