Investigation Summary
At approximately 12:00 p.m. on July 19, 2001, Employee #1 was working at the rear of a private residence preparing a window box for painting by manually sanding it with sandpaper. Employee #1 was standing on the fourth step of a Werner eight-foot portable aluminum stepladder. The rear two legs of the ladder were placed on cement while the front legs were placed on loose soil next to the house. The left front leg of the portable ladder sank into the loose soil and the ladder tipped over causing Employee #1 to fall to the ground fracturing the "ball" of the ball-and-socket joint of his right hip. Employee #1 was hospitalized.
