Investigation Summary
Employees #1 and #2 worked for a commercial roof and floor construction company. They were setting roof panels in place and standing on top of a wood roof panel. They were fitting the roof panel in place by wedging and hammering the hanger at the end of the purlin (both part of a wood panelized roof system) to get it to fall into place on the ledger. Employee #1 and #2 fell approximately 27 ft through the roof to the floor. Protective systems were not in use at the time to protect them. A hanger was observed on the ground directly below showed signs of bending and wears. After the accident, the employees were hospitalized and treated for their injuries.
