Investigation Summary
Employee #1, who worked for a clothing recycling operation, was using a forklift to empty a small bin of clothing into a larger one. Following normal procedures, he raise the small bin on the blades of the forklift to a height of approximately 7 ft and then drove the elevated bin over to the edge of the large bin. Employee #1 then exited the vehicle and stepped up on its left side to release a mechanism that dumped the contents of the small bin into the large one. Apparently Employee #1 was standing between the mast uprights and the rollover cage when he released the mechanism and became pinned between the frame and forks of the lift truck. He suffered injuries to the pelvis and back and was transported to Martin Luther King Hospital. There were no witnesses to the accident.
