Investigation Summary
An employee was injured at a company engaged in packaging and distributing newspapers. The accident occurred at approximately 7:00 a.m. on August 14, 2000. It involved a 53 year old male material handler/forklift driver at a newspaper insert line. The injured employee observed a newspaper jam on one of the company's Dynaric, Inc. plastic strapping units. In order to clear the newspaper jam in the machine, the employee switched the machine to manual mode and squeezed between the space adjacent to the control panel to clear the newspapers. Once he cleared the newspapers, he began to line them up so that the machine could place the plastic strap around the bundle. The employee accidentally depressed the start button on the machine causing the machine to cycle. This activated the heating/cutting element used to cut the plastic strapping. His hand was beneath the bundle of papers he was aligning. The tip of his left index finger was amputated by the heating/cutting element of the strapping machine.
