Investigation Summary
At approximately 8:30 a.m. on December 17, 2001, at the time of the accident, Employee #1 was working alone on the fourth floor of a multi unit wood structure apartment complex, which was under construction. He was assigned by his foreman to run conduit through wall studs and floor and ceiling joists using a 0.5-inch Milwaukee Hole Hawg with an 18-inch by 1.125-foot Milwaukee auger. While Employee #1 was drilling the auger bit (which has razor sharp edges) became loose in the chuck. He attempted to tighten the chuck by placing the end of the auger perpendicular to the floor, holding the auger with his left hand, the chuck key in his right hand and the Hole Hawg between the his legs. As he began to tighten the chuck the trigger was activated by his leg causing the auger to turn, catching the middle finger in the auger flight, causing the amputation. Employee #1 stated he forgot to unplug the Hole Hawg prior to adjusting the chuck. He suffered the amputation of the distal left middle finger tip at the first knuckle. Employee #1 was transported to Long Beach Memorial Hospital by the Long Beach Fire Department Paramedics. He was hospitalized for his injuries.
