Investigation Summary
At approximately 7:15 a.m. on August 15, 2001, Employee #1, working at an aluminum manufacturing company, was removing a small piece of masking tape from a roll on an aluminum sheet coating/painting line. Employee #1 was the head operator on the rewind (or payoff) end of the line, where his job is to monitor the process, including quality control. During the run, the finished product was flawed with a small ding, which meant that a foreign object was on one of the rolls in the process. The operators were able to determine the location of the cause of the ding, but the line was not shut down. Employee #1 left his position and went to the problem roll located on the west side of the accumulator derrick. The employee opened the unlocked gate and proceeded towards the problem roll. The gate on the west side was not interlocked to shut down the line when opened. However, the gate on the east side is interlocked and will stop the line. Employee #1 saw a piece of tape near the center of the roll. As he reached in to remove the tape, his arm was caught and torn out of his body, along with chest, neck and back muscles. Employee #1 was transported to Harbor General Medical Center by Los Angeles County Paramedics where he was treated for an amputation of the left arm at the shoulder.
