Employee Injured Due To Ladder Collapse

At approximately 10:00 a.m. on July 19, 2001, Employee #1 was assisting with heating, ventilation and air condition installation by applying the insulation materials and sealers to the ducts. Employee #1 set a ladder against a ceiling-metal bar to reach the ducts located overhead. Employee #1 then ascended the ladder without first tying or securing the ladder to prevent it from displacing. Employee #1 fell when the 20-ft extension ladder collapsed. Employee #1 suffered a compound fracture to his left leg and was hospitalized. .
