Employee injured in fall from warehouse mezzanine

At 3:30 p.m. on June 20, 2001, Employee #1, a grocery worker was working alone unloading boxes of dried noodles from an elevated pallet on a fork lift to the mezzanine. Employee #1 had tried to unload a box from a pallet that was located toward the mast of the fork lift. The pallet was not readily reachable from the mezzanine. To reach the box, Employee #1 positioned one foot on the mezzanine and the other on the elevated pallet. Employee #1 fell 8 feet, 2 inches to the concrete floor from the mezzanine in the warehouse, causing serious internal head injuries. Employee #1 was transported to the hospital, where he was hospitalized and treated for a concussion. The mezzanine did not have guardrails and personal fall protective devices were not available.
