Saw Blade Punctures Employee'S Chest

Employee #1 had lacerations and punctures in his chest when the saw blade he was using broke and penetrated his chest. The machinery used was a Joint Cleanout Saw manufactured by USF, with Model Number JS 90. This saw has a Sawtec 12-inch diameter dry diamond blade running at approximately 2,400 r.p.m. His employer repairs concrete slabs and cleans concrete joints. His employer failed to perform routine maintenance and inspections on the saw.
