Investigation Summary
Employee #1 worked for a manufacturer of high loft, non-woven, synthetic fibers such as polyester. He was guiding fabric through the #1 fiber-bonding (garneting) machine on line #2 when he slipped. As he tried to maintain his balance, he reflexively grabbed onto the chain and sprocket drive, amputating the four fingers on his left hand and lacerating his right hand. The chain and sprocket drives in the area were approximately 5 ft 7 in. from the floor, and were not guarded.
