EMPLOYEE INJURED WHEN STRUCK BY GATE

At approximately 5:50 a.m. on March 19, 1998, Employee #1 had parked his loaded truck outside the warehouse facility and walked back to close the yard gate. While pushing the sliding half of the gate (24 ft long by 10 ft 2 in. high) to its closed position, the gate went beyond its designated stopping point. Apparently, the L-shaped stop bracket was missing from the rear of the sliding gate. Consequently, the gate no longer had support at the top and it began to fall on Employee #1. Employee #1 tried to keep the gate from falling but steel gate was too heavy. Employee #1 sustained a fracture of his left arm and other bodily injuries and was hospitalized for treatment.
