Investigation Summary
At approximately 9:04 a.m. on September 18, 2008, Employee #1, a winery worker, was cleaning out a press. The press has an auger, 12 in. in diameter and approximate 8.5 ft long. The end of the drain is approximately 5.625-in. from the floor and the length of drain pipe is approximately 1.125-in. The auger was still rotating when he stuck his fingers in the drain on the underside of a tank to clear it. The auger amputated two finger tips.
