EMPLOYEE #1 WAS PERFORMING ROOF REPAIR OPERATIONS ON A BUILT UP ROOF SURFACE. HE WAS UNROLLING THE ROOFING FELT BY WALKING BACKWARDS WITH THE ROLL IN HIS HANDS. HE BACKED ONTO A PLEXIGLASS SKYLIGHT AND SAT DOWN ON IT. THE SKYLIGHT GAVE WAY AND EMPLOYEE #1 FELL 23 FEET TO A CONCRETE FLOOR. HE WAS KILLED.
