Employee killed when crushed by large mining equipment tire

Employee #1 was removing a large mining equipment tire in preparation for shipment. The tire/wheel assembly was on an oscillating axle, which allowed the tire to lean toward him as he worked. Using an air gun, Employee #1 inadvertently removed the last lug nut and the tire fell on him. He was crushed and killed.
