Employee killed when struck by hose clamp fitting

Employee #1 was operating a small, coal bed methane drilling rig at a drilling operation that began encountering circulation problems shortly after another section of pipe was introduced. He throttled up the rig seconds before a 3 in. ID boss-type clamp fitting on the mud line pressure hose burst loose and struck him in the forehead. He sustained severe a puncture injury and died within an hour of the accident.
