Investigation Summary
Employee #1 and coworkers were assembling the T-bar, or top section, of a 1000 ft antenna on the ground when it was noticed that one section of the T-bar was incorrectly put together. Connecting bolts and structural members were removed to correct the problem. A section of the antenna weighing several thousand pounds fell, trapping and killing Employee #1. It is believed that a gust of wind caused the section to fall.
