FALL FROM ELEVATION

An employee was on a vessel afloat working on a 254-millimeter-diameter steel pipe that was about 2.7 to 3.0 meters above the deck. This pipe was being installed on top of a 711-millimeter-diameter pipe using three stanchions. One of the stanchions on the end had been tack welded to the larger pipe. The smaller pipe was being pulled over into position by a come-along. The employee was cutting on the middle stanchion when the smaller pipe broke loose and threw him onto a pipe on the dock. He then fell into the water and died. The employee was not wearing a safety belt with a lifeline or a life vest.
