Employee killed in fall from roof

On September 17, 1997, Employee #1, a maintenance worker, was changing some belts on an air blower that was located on the roof of a building. He traveled back and forth twice between the roof and the ground floor of the building, each time using a 25 ft caged ladder with a counterweighted hatch cover, which was located near the center of the roof. For some reason, on his third trip up, Employee #1 decided to ascend the exterior of the building. He asked a coworker to use a large fork truck to elevate him to the rear corner of the roof while he stood on the forks. Employee #1 was on the roof when, minutes later, he fell 25 ft to the ground, landing very close to the fork truck. He sustained head injuries and died on September 19, 1997.
