Investigation Summary
Employee #1 was using a chain saw to fell trees in the woods. He was wearing a hard hat, mesh screen for eye protection, steel-toed shoes, and blue jeans, but no shirt. As he felled a large diameter tree, the branches from that tree became entangled in a tree about 12 ft away. A large dead branch was dislodged from the upright tree, and it fell on Employee #1. He was retreating from the tree he had cut and did not see the large branch that struck him on the head and knocked off his hard hat. He was transported by emergency services to the hospital, where he was pronounced dead.
