Employee dies after fall from ladder

On November 10, 1997, Employee #1 was on a 10 ft wooden A-frame ladder using a double-insulated Bosch demolition hammer to make a 5 in. diameter hole in the upper portion of a wall prior to installing some plumbing pipes. Employee #1 fell from the ladder and struck his head on the concrete floor. He suffered a fractured skull and he was pronounced dead the next day.
