Employee struck and killed by pile driver

Employee #1, a foreman, was preparing the dirt surface for a 7 in. diameter by 19 1/2 ft long steel pipe that was to be driven in by a pile driver. The operator of the Manitowac 2900 crane was positioning the pile driver in the same spot as Employee #1 when the hammer unexpectedly fell. It struck and killed Employee #1.
