Investigation Summary
At approximately 11:30 a.m. on October 1, 2000, Employee #1 and coworkers were placing sheet metal, insulation, and chicken wire on the roof of a metal warehouse under construction. Employee #1 slipped from the roof and fell inside of the building, striking his head on the concrete floor. The ambulance arrived immediately and the paramedics stabilized Employee #1. He underwent emergency surgery at the hospital, but died the following morning.
