Investigation Summary
On September 2, 1998, Employee #1 and Employee #2 were aligning a 24-inch gas pipe. The gas pipe came out of an alignment clamp and struck the Employees. Employee #1 was struck in the chest. Employee #2 was struck in the hip. Employee #1 was killed. Employee #2 was hospitalized for a broken hip.
