Investigation Summary
Employee #1 was packing headrests into boxes near a forklift traffic area. A forklift carrying collapsed metal racks was traveling by her when the unsecured load on the forks shifted. The racks fell, striking the box on which Employee #1 was working; she was then struck by the box and knocked to the ground. Employee #1 sustained a fractured coccyx, along with a strained neck and wrist.
