Employee killed by falling load of glass

Employee #1 was using a forklift to unload cases of glass from a truck. The cases on the forks began to teeter, and he jumped from his operator's position to the ground in an attempt to prevent them from falling. Employee #1 was crushed against the truck bed by the falling cases of glass, and was killed.
