Employee killed in excavation cave-in

Employee #1 and a coworker were installing five water well curbings in an approximately 18 ft deep excavation. One curbing had been installed, and the second was being installed by a trackhoe operator when a wall of the excavation caved in. Employee #1 was buried by the collapsed wall of dirt and was killed. The coworker escaped. The excavation had vertical walls, with the end sloped as a means of egress.
