Investigation Summary
On June 23, 1997, Employee #1 and two coworkers, of Crocker Crane Company, Inc., were dispatched to fix a derailed boxcar in Saginaw, TX. They were to use a crane mounted on rail wheels to right the boxcar. The employees placed the crane on the rail tracks about one-half mile west of the boxcar. They then boarded the crane and rode it under its own power to the boxcar. One coworker was in the cab operating the crane and Employee #1 and the other coworker were standing on a 2 ft wide platform in front of the crane. The crane had a maximum speed of 6 mph and the operator said it was maneuvering at about 3 mph. With the employees riding in front of the crane was a coupler yoke, a 100 to 200 lb steel cable hoisting mechanism. It began to slip off the platform while the crane was in motion, and Employee #1 gave the operator the signal to stop. While the driver was slowing the crane, Employee #1 reached down, apparently to retrieve the yoke or to keep it from falling. He fell from the platform and was crushed under the platform support plate before the crane came to a halt. Employee #1 was killed.
