Investigation Summary
At 3:00 p.m. on July 10, 1998, Employee #1 and two coworkers were on a fabricated frame scaffold, finishing the grout on the inside of a 30 ft tall hollow block wall. Employee #1 was at the end of the scaffold system at the northeast guardrail post, preparing to descend an extension ladder set up against the scaffold. As he went over the guardrail to access the ladder, the post detached from the scaffold and Employee #1 fell 26 ft to the concrete foundation. He was killed. The guardrail post was defective, cracked, and not properly installed to the coupling pin inside the end frame. Apparently, the toggle pins were either missing or not engaged.
