Investigation Summary
Approximately 10:00 a.m. on March 29, 1997, Employee #1 was refinishing a wooden boat in the repair shop of his employer. He was standing on planks atop drums that were being used as a makeshift scaffold when he lost his balance and fell to the floor, injuring his head, shoulder, and knee. Employee #1 was taken to the hospital, treated, and released. He returned to work on March 31, 1997, but died on May 12, 1997, as a result of the head injuries received in the accident.
