Investigation Summary
At approximately 11:00 a.m. on June 20, 1997, Employee #1 and two coworkers, a forklift operator and a laborer, were using a forklift to load bundles of lumber into a transport container. Three bundles were being moved when the load shifted, causing one of the bundles to fall. It struck and pinned Employee #1, who had entered the container unbeknownst to his coworkers. He sustained chest injuries that resulted in his death. Other employees verified that Employee #1 had been instructed not to enter the container during the loading process.
