Employee killed when struck by forklift load

At 3:00 a.m. on November 29, 1996, Employee #1 stepped in front of an industrial truck that was traveling forward with a load of paper. The truck was able to stop but the load of paper kept going, knocking Employee #1 to the concrete floor. He suffered a fractured skull when his head struck the floor. Employee #1 was killed.
