Employee killed when crushed between valve and pipe spool

At 10:15 a.m. on October 12, 1996, Employee #1 was rigging up a 24 in. valve that was to be lifted off a spud barge and onto a concrete platform. Employee #1 gave a verbal signal to the foreman, who then relayed instructions to the crane operator to move it up a little. When the 20,000 lb valve was lifted about 1 in., it swung outward enough to contact another pipe spool on the deck. For some reason, Employee #1 had placed his head between the valve and the pipe spool, and his skull was crushed. He was killed instantly.
