Employee killed in 45 ft fall from tree

On September 7, 1996, Employee #1 was practicing for certification in single-man tree rescue technique in Sulphur, LA. Other employees of Wireless One, Inc., were also at the site, along with two instructors. There had been 18 hours of instruction and demonstration covering how to use the safety equipment to rescue a dummy in a magnolia tree at the site. Prior to Employee #1's ascent, the instructor had checked that the snap hooks on Employee #1's safety belts had closed properly. When Employee #1 reached a height of approximately 45 feet, he was seen to be tied off with a safety line and a snap hook with a screw-type closing. He had his back to the observers on the ground and was preparing to make his descent with the dummy when he unexpectedly disconnected his safety line and fell from the tree. Employee #1 was killed.
