Employee killed in fall when ladder slips

Employee #1, a Cablevision cable splicer, was using a 28 ft fiberglass ladder to work on some coaxial cables. A construction crew had installed a 1/4 in. steel cable and a 3/4 in. coaxial cable, and Employee #1 was going to splice the unconnected ends and then run cable to individual homes. He did not set the ladder on the steel cable correctly and when he was almost to the top, the ladder began to feel unstable. Employee #1 was descending when the ladder slipped and he fell approximately 14 ft. He suffered a fractured lumbar vertebra and was taken to a hospital in New Orleans, where he died 2 days later. A blood clot was suspected to be the cause of death.
