Employee killed in fall when ladder overturns backward

Employee #1 was on a 12 ft ladder, guiding the top wheels of a sliding glass door back on their 16 ft high track when he overreached the ladder's center of gravity. The ladder kicked away from the building and Employee #1 fell forward toward the building. He landed on his face in an asphalt parking lot, and was killed.
