Employee struck by industrial truck is killed

On October 21, 2003, Employee #1, a truck driver, was parked at a hospital loading dock unloading plastic trash containers by, hand from within the box trailer. He was having difficulty moving some of them because a stack of them fell over and he could not upright the load. A fork lift driver told him that he would help him stand the stack of containers up with the fork truck. The fork lift truck driver drove into the trailer, with Employee #1 in the trailer. The fork lift truck driver tried to stand the load up. When he was about to back away from the standing load, Employee #1, who was standing in front of the raised forks, was struck on the right upper torso. The operator, who thought that the fork truck was in reverse gear, gave it power to leave. The truck lunged forward and struck Employee #1, killing him.
