Investigation Summary
On December 12, 2002, Employee #1 and coworker were realigning pieces of equipment on a low flat bed truck to better distribute the weight and to remove a piece so that the truck would not be overweight. The plan was to raise this 15,000 pound machine with the fork lift and then back the truck up about 4 feet to lower the machine. The machine hung up on another piece of equipment and the crew raised it a little and told the driver to back up a little. The truck was on a slight incline and when the driver let off the brake and put it in gear, the truck started to shake. The fork lift operator felt the fork lift shaking and suddenly the floor in the bed of the truck broke. The press broke free of the other machine and started to move. Employee #1 ran between the machine and fork lift and tripped over the forks. The machine hit him in the back of his head and then fell on him, killing him.
