Investigation Summary
At 2:15 p.m. on July 29, 1996, Employee #1 was raking trash in a residential area where some trees were being cut. The employer warned Employee #1 that a 17 ft 3 in. tall tree was about to be felled. At that time, Employee #1 was 16 ft 5 in. away from the intended fall line of the tree and acknowledged the warning. However, when the employer completed the cut and looked up, he saw Employee #1 had for no apparent reason moved directly under path of the falling tree. Employee #1 was struck and killed.
