Investigation Summary
On September 23, 2001, Employee #1 was part of a crew of the KVP Falcon Plastic Belting Inc., a manufacturing company that was installing plastic conveyor belting on a new spiral conveyor. The conveyor was being installed as part of a new manufacturing line in a food processing plant. One employee was directed to ride inside the rotating structure of the spiral conveyor so he could watch the belt as it was being loaded. He could also make adjustments to a rope that was being used to pull the belting onto the conveyor track. Employee #1 climbed up to the top of the conveyor while it was in motion. Employee #1 was crushed between a stationary support member and one of the rotating spokes of the conveyor. Employee #1 was killed.
