Investigation Summary
Employee #1 entered a 31 ft diameter by 89 ft tall Harvestor silo through a 17 in. diameter opening in the top to replace a bladder bag. The atmosphere in the top of the silo was oxygen deficient, and Employee #1 was overcome. He died of asphyxia.
