Employee killed when run over by dump truck

Employee #1 was walking on a graded surface toward a dump truck that was backing up to a skimmer machine. The dump truck ran over him with its right rear tandem wheels. Employee #1 was crushed and killed. The truck was equipped with an operational back-up alarm.
