Investigation Summary
Employee #1 was using a Wrangler front end loader to bring ventilation pipe into a grain flat storage building. He was driving under an end wall support rod when he was knocked from the operator's seat and his knee landed on the reverse hydrostatic control pedal. Employee #1 became trapped between the support rod and loader firewall, with the loader operating at full power in reverse. Employee #1 died within several hours of injuries sustained in the accident.
