Employee killed in 14 ft fall from tank

Employee #1 was approximately 14 ft above the floor, cleaning the top of a production run tank. He either slid off the top of the tank or fell when exiting the tank onto an unsecured 28 ft fiberglass extension ladder. He landed on the concrete floor and was killed. Employee #1 was not using fall protection. There were no witnesses to the accident.
